What Are Home Care Packages (CHCP)?

Home Care Packages (CHCP) are government-funded services designed to provide personalised support to older Australians, allowing them to live independently in their own homes for as long as possible. These packages cater to individual needs, offering a wide range of care and services to enhance your quality of life.

Who is eligible?

CHCP is available for older Australians aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander people) who need coordinated support for daily living activities.

CHCP Levels Tailored to Your Needs

Home Care Packages are available in four levels, reflecting the level of care and support you require:

How to Access CHCP

To access CHCP services, you’ll need to follow these simple steps:

  • Contact My Aged Care: Call 1800 200 422 or visit My Aged Care to register.

  • Assessment: Have a face-to-face assessment with an Aged Care Assessment Team (ACAT) to determine your needs.

  • Approval: Receive confirmation of your HCP level and funding.

  • Choose Kalang Community Care: Partner with us to develop a personalised care plan and start receiving services.
